Photo guestbook with prompts/topics

I was hoping someone could help me with this..

 

I found this on Pinterest (see below), a guestbook photobook with different questions/sayings on each page where guests put an answer instead of just signing. I love this idea and this particular one was done by an Etsy designer and she is no longer making them. 

 

Does anyone know where I could find something like this? Not sure if this would be easy to make with a normal photobook like with Shutterfly or something.

I made one on Shutterfly recently... They do have some templates in their wedding books that are similar... The pages I did:
Wedding Mad Libs

In 5 years from now.... (where do you see us?)

We belong together because.... (tell us what you think!)

 

There are a few other idea pages they have that are cute. I did a few repeats so people have options if one of those was already filled out. There are also options for standard pages where people can sign in between pics of you, etc. I will see if I can post some pics of mine

This is a really cute idea!!  I would recommend mypublisher, as I think their quality is far better.  And remember the amount of guests that you'll have vs page numbers and sizes.  You don't want only one, or even no signatures on a page.
