Papermart has a large variety of boxes - all sizes shapes & colours. I know a lot of people who have had good experiences ordering from them. Their prices are pretty reasonable too. Paper Mart Packaging Store - Apparel Boxes & Gift Boxes -

I had four inserts plus the invite itself which were all custom sizes. I created each one in Publisher and then brought the files to a local printer (not a Staples/Kinkos, just an independent printer) and gave them the finished dimensions for each insert. I also printed/cut out 1 sample set myself to give them for reference. They printed and cut each one to size & I think it cost me around $250 total for 150 invites. I felt that it was worth the cost to save me the hassle.

Hi Everyone, My name is Stacey and I recently got married (May 3) and did a lot of my own stuff (STDs, invites, seating chart, etc). I have now offered to do the same for my little brother's April/09 DW in Mexico, and I thought I'd come on here to get some ideas. So far I have seen a lot of great ones!
