I too am American and married a Brit, but we met while working in the Turks and Caicos so we had destination weddings in Virginia and the UK (both very small)LOL as we play on the beach all of the time. 13 years later it's all good.

 

Our island (Providenciales, Turks and Caicos) is fairly quiet still and we do have an intimate feel and great hotels and restaurants. It is pricier than mexico/bahamas/central america, but the currency is the American dollar which is helpful. Most hotels are actually high end condo/self catering style. Because we still don't get huge amounts of tourists we are off of a lot of radars when people are checking things out. It may be the type of place that could interest you. In addition to Provo (the main tourist island) there are smaller cays that have private resorts if you really want to splurge. My husband said he is really surprised at all of the resorts that are encouraging weddings now and offering packages (which hasn't always been the case)

 

Belize is also nice (Ambergris Cay is my favorite place there), and I have been to a wedding on the western coast of Costa Rica that was intimate (there were 6 countries represented from all parts of the globe and travel wasn't easy though). Hubby recently was in the Bahamas at a small resort that ended up being a really nice small wedding with many international guests from 5 different countries (that was easy to fly into). The Caymans are also pretty, with lots of choice (a bit busier than we are)

 

You'll get some great advice here, figure out whats most important to you and FI and let the rest fall into place.

I too spent hours trying to decide on a location. After three weeks of researching my dream wedding in Aruba- and five minutes before booking a trip for a site visit I switched locations completely. Now I am going to Cabo and I am so excited.

What made me change? Easier travel for family on the West Coast and friends in the East coast. And it is awesome!
